You’ll need to allow around 45 minutes to fill it in.

This is the perfect opportunity to try out that new tea you bought and spend some time focusing on you.

Some of the fields are required (those with stars next to them), but for the rest just fill them out to the best of your ability. If there is something that you are not sure of, we can talk about it together.

There are no wrong answers, and no judgement! Answering as honestly as possible will allow me to serve you more effectively.
